Your Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, will be available for you to access electronically, you can view and print it in Workday as follows:

繚 Select the Benefits and Pay Application from the Menu
繚 Click on Benefits and select My ACA Forms
If you are unable to access your form in Workday and would like to request a paper copy, you may do so by:
